I’ve been using Kodak Digital GEM Airbrush Pro for skin cleanup and texture smoothing, but it appears there’s no CS5/Mac/64-bit update, and I don’t want to run Photoshop in 32-bit mode. Aside from Kodak GEM and Portraiture, what plugin options are available for airbrushing or skin-smoothing that support Photoshop CS5 64-bit on Mac?

So it turns out there is Topaz Clean by Topaz Labs. Cheaper, supports CS5 64bit and it can also add texture. The airbrushing, however is not as good as the Kodak's, but since it's used only with a small opacity, it will do.

One option mentioned is Topaz Clean from Topaz Labs. It supports Photoshop CS5 64-bit on Mac, is less expensive than some alternatives, and can also be used to add texture.
The tradeoff is that its airbrushing/smoothing effect isn’t as strong as Kodak Digital GEM Airbrush Pro, so if you prefer the Kodak look you may find it a step down. However, the community feedback suggests it still works well when applied subtly at low opacity, which is often a good approach for natural-looking skin retouching anyway.
So based on the answers provided, Topaz Clean is the main recommended alternative for your setup.
