Why is Canon 60D Live View darker in Manual mode than the final photo?

On a Canon 60D, Live View looks correctly exposed in Tv mode, but in Manual mode the preview appears much darker even though the photo playback looks normal. This happens while using a manual lens that reports f/00, with the same ISO and shutter settings. Exposure simulation is enabled. What camera settings can cause Live View exposure simulation to look inaccurate in Manual mode?

You need to be sure "Exposure Simulation" is enabled.
The setting is found in the fourth 'Function' tab of the camera's menu. Since all of the settings for this tab are for use with Live View, this tab is only displayed in the menu when the camera is set to Live View mode. The fifth setting from the top is 'Exposure simulation'.
If you have 'Auto Lighting Optimizer' enabled, the simulated exposure may not be correct. Set 'Auto Lighting Optimizer' to 'Disabled'.
If you have 'Highlight Tone Priority' enabled, the simulated exposure may not be correct. Set 'Highlight Tone Priority' to 'Disabled'.
