How can I protect my camera and lens while shooting in rain or snow?

I shoot in a very rainy and snowy climate and want to keep using my camera outdoors without moisture building up on the camera body or lens. I already use a camera bag and the usual sealed-bag method when moving between temperatures to reduce condensation. One of my camera/lens combinations is weather sealed, but much of my gear is not. What can I use while actively shooting to protect the camera from rain or snow accumulation?

What about a rain sleeve like the one shown here at Amazon? Very cheap solution to your problem...
