How can I keep a BlackRapid shoulder strap from slipping off while shooting?

I use a BlackRapid Sport camera strap and it is comfortable while the camera is hanging. The problem is that when I raise the camera to shoot, the shoulder pad can slide down my arm because the camera’s weight is no longer holding it in place. I have tried adjusting the under-arm strap both tight and loose, but it still slips. Is this normal, or is there a better way to wear or adjust the strap so it stays secure while shooting?

If you wear it as intended, or even not quite as intended

Then there's the possibility it can fall off.

If you wear it not as intended, over your head

then it has no escape.
