In Lightroom 5 Smart Collections, I can add multiple rules and choose whether to match "any" or "all" of them. Is there a way to group rules so I can build logic like: A must be true, and at least one of B, C, or D must also be true?

Yes, you can do nested conditions.
When in the window for defining a smart collection, hold down the Alt (Windows) or Option (OSX) key, and the + button on the right for adding conditions will turn into a # button. Click it, and you can create nested conditions.

Yes. Lightroom 5 Smart Collections support nested condition groups.
When you’re editing the Smart Collection rules, hold down Alt on Windows or Option on macOS. The + button used to add a rule changes to a # button. Click that to add a nested group of conditions.
That lets you build logic such as:
- A is true
- AND
- any of B, C, or D are true
So the kind of grouped rule set you described is possible in Lightroom 5.
