I want to set up a Canon EOS Rebel T5i to shoot a long-term timelapse at about one photo per minute for roughly a month. Can a Wi-Fi SD card be used to automatically send images to an external location such as a PC, or is there a better setup for this kind of extended unattended shooting?

Since your camera is going to need to be powered from an external source for a time period covering anywhere approaching a month, you might as well also connect it directly to a computer via the USB port and use tethered shooting.
