Why is the circular fisheye image off-center on my Sony A7R III with a Canon 8-15mm and adapter?

I'm using a Canon EF 8-15mm fisheye on a Sony A7R III with a Sigma adapter. At 8mm, the circular fisheye image isn't centered in the frame and sits closer to the top edge. Is this more likely to be a lens defect or an adapter/alignment issue?

The most likely cause is an alignment issue that results in the lens' optical axis not being perpendicular to the camera's sensor. I'd place money that the adapter is not sitting perfectly flat with either the camera's flange, the lens' flange, or both.
