Why does my Canon 5D Mark II battery lose charge while the camera is off?

My Canon 5D Mark II loses battery charge even when switched off. Overnight it can drop by about 15%, and if I leave a battery in the camera for longer, sometimes the camera will not power on at all. I’ve tried removing the lens and memory card, and the camera has no GPS or battery grip. Firmware is 2.1.2, and I also removed both the main battery and the small internal battery to reset the camera. If I leave the battery out of the camera overnight, it shows no charge loss. What could be causing this?

As per comment these batteries are very old and with high probability they can't keep the charge for long time. This is normal behavior for Li-ion, they have around 500 cycles of charge.

The best recommendation is to buy new batteries and replace existing. Just forget about old batteries and find appropriate way to dispose/recycle them.

The most likely cause is aging batteries rather than firmware or accessories. Older lithium-ion camera batteries lose capacity and can no longer hold charge well, especially after many charge cycles. A commonly cited lifespan is around 500 cycles, after which noticeable deterioration is normal.

If your batteries are old, they may appear to charge to 100% but then drop quickly or fail to deliver enough power when left in the camera. The practical fix is to replace them with fresh batteries and recycle the old ones properly.

Since the batteries do not seem to self-discharge when stored outside the camera, there could be a small drain from the camera body while off, but healthy batteries should usually tolerate that much better. Start by testing with a known-good new battery before assuming a camera fault.
