How do aperture and ISO work when using Bulb mode?

If a camera is set to Bulb, the shutter stays open for as long as you hold the release, so the camera cannot know the exposure time in advance. In that situation, how are aperture and ISO determined? And if a camera allows Bulb to be selected outside full Manual mode, what does it do for the other exposure settings?

Most of the cameras that I know have separate Bulb mode and shutter priority, the Bulb can be in the manual mode and you can get it by increasing the shutter speed till you get bulb (Like in some Canon models). But it's not reasonable to make it attached with the shutter priority mode because the camera wouldn't know in advance how long you are going to open the shutter! The camera won't meter properly then and adjust the aperture and ISO?

On most cameras, Bulb is essentially a Manual-mode shutter setting. The camera does not automatically determine exposure because it cannot know beforehand whether you will hold the shutter open for 5 seconds or 30 seconds.
In normal Bulb use:
- shutter time: controlled by you

- iso: set by you
So Bulb is usually the most manual exposure mode available.
