How can I reorganize Lightroom photo folders without breaking the catalog?

I have an older Lightroom catalog that imported photos into one folder per day (for example, 2006-01-07) under a single main directory. I’m now considering reorganizing everything into a year/month structure such as 2006/01/07 or possibly just 2006/01.
All of the photos are already cataloged, and my RAW files also have sidecar XMP files. I’m using Lightroom 4.
Do I need to create a new catalog and re-import everything, or is there a better way to move folders and photos into a new structure while keeping Lightroom’s database intact?

You can reorganize folders quite easily within Lightroom -- just drag and drop. The trick is simply to do all of the work in Lightroom so that it understands where you're moving photos to and can keep the database updated. (That is, do not go to Finder or Explorer and start dragging folders around.)
In the Folders panel in the Library module, I'd go about this by creating the folder structure 2006/01 then dragging 2006-01-07 into it -- creating 2006/01/2006-01-07. A little redundant, but very fast and easy!

You generally do not need a new Lightroom catalog. The safest approach is to reorganize the folders from within Lightroom so the catalog always knows where the files are.
Use the Folders panel in the Library module to create the new year/month folders, then drag existing day folders or selected images into that structure. For example, create 2006/01 and drag 2006-01-07 into it, or move all images from that month into a single month folder if that’s your preferred layout.
The key point: do the moves inside Lightroom, not in Finder or Windows Explorer. If you move folders outside Lightroom, the catalog can lose track of the files and you’ll have to relink them.
