In Lightroom, some thumbnails in Library show a question mark, meaning the photo is missing or offline. I’ve seen this happen to only a few images within a folder, and with thousands of photos in my catalog it’s hard to locate them again manually. Is there a built-in way to list or filter all missing photos, similar to filtering by flag, rating, or color label?

In the Library module, try Library -> Find All Missing Photos. I have LR 5, so I know it's in there. I don't know about LR 4.

Yes. In the Library module, use Library → Find All Missing Photos. Lightroom will gather the images it currently marks as missing so you can locate and fix them, rather than searching folder by folder.
