I’m trying to recreate the blurry duplicated/ghosted look where a person appears repeated off to one side of the frame. Is this typically done with an app or Photoshop, or can it be created in-camera? What kind of filter or accessory would produce this effect?

A glass prism would create this effect.
A similar type of filter is known as kaleidoscope filters.

This effect is commonly created in-camera, not with an app. A small glass prism held in front of the lens can refract part of the scene and produce a soft duplicated or ghosted copy of the subject off to one side. Similar accessories are often sold as prism or kaleidoscope filters.
