I want to use a 256GB microSD card in a Canon EOS 5D Mark III. Since the camera has an SD card slot rather than a native microSD slot, I’d be using a microSD-to-SD adapter. Will the camera recognize and format a 256GB microSD card this way, and is there any known capacity limit for the SD slot?

Yes — a 256GB microSD card should work in the Canon EOS 5D Mark III if it’s used in a microSD-to-SD adapter.
The 5D Mark III supports SD, SDHC, and SDXC cards in its SD slot, and microSD cards are not used directly unless adapted to full-size SD. A passive microSD-to-SD adapter does not normally reduce speed by itself, since it mainly just maps the contacts.
Based on the community reports, 256GB cards have been successfully used, formatted in-camera, and tested for RAW/JPEG shooting and transfer to a computer. Canon documentation also indicates support for cards above 128GB, which aligns with SDXC support.
So the practical answer is: yes, a 256GB microSD card can be used via an adapter in the 5D Mark III.
