I'd like to recreate an old, faded photo effect similar to vintage film prints using a photo taken on my smartphone. Is this possible in Photoshop, and what adjustments or tools should I look for to get a warm, slightly desaturated, lower-contrast result?

An easy way to get film looks in Photoshop is to use the Analog Efex free plugin: https://www.google.com/nikcollection/products/analog-efex-pro/ With some experimentation with different film types you should be able to get something close with minimal effort.
